ABSTRACT

OBJECTIVE: To establish an effective clinical pathway of hospitalization process for occupational chronic mild lead poisoning within the framework of project time; to formulate a rational and effective management model for the diagnosis and treatment. METHODS: Occupational chronic mild lead poisoning was selected as a disease for pilot study based on GBZ 37-2015 Diagnosis of Occupational Chronic Lead Poisoning. According to evidence-based medicine criteria,the clinical procedure was developed,preliminary used and evaluated in clinical practice in several hospitals,with the best treatment scheme of the disease. The principal methods used are literature research,case analysis and expert consultation.RESULTS: Standardized documents relevant to clinical pathway for occupational chronic mild lead poisoning have been formulated,including standardized in-hospital treatment process,Clinical Pathway Forms for medical staff and Consent Documents of Clinical Pathway for Patients. Preliminary clinical research showed that this clinical pathway is conducive to standardizing medical process,improving medical quality,shortening the time in hospital,reducing medical cost and the waste on medical resources. CONCLUSION: The results confirm that the clinical pathway for occupational chronic mild lead poisoning is scientific,rational,normative and practical. It will surely contribute to promote the management of disease diagnosis and treatment.

ABSTRACT

OBJECTIVE: To explore the clinicopathological characteristics and therapeutic approach for mercury poisoningassociated nephritic syndrome. METHODS: Clinical and pathological data of 9 patients diagnosed with mercury poisoningassociated nephritic syndrome were analyzed. RESULTS: Among the 9 cases,2 cases were mercury smelters,1 case was instrument production worker,1 case was traditional Chinese medicine preparation worker,4 cases were psoriasis treated by folk prescription of traditional Chinese medicine,and 1 case used cosmetics for whitening and freckle removal. All cases showed clinical manifestation of nephritic syndrome that included large amount of proteinuria,hypoalbuminemia,edema and hyperlipidemia. The concentrations of urine mercury in the patients were from 0. 09 to 0. 75 mg / L before treatment.Pathology examination on renal biopsy showed that there were 4 cases of minimal change nephropathy and 5 cases of membranous nephropathy at stage Ⅰ. The patients with nephritic syndrome were cured after mercury driving and treatment with corticosteroid. CONCLUSION: The kidney histopathology changes in patients with mercury poisoning-associated nephritic syndrome are primarily characterized by minimal change and membranous nephropathy. The mercury driving combined treatment with corticosteroid can completely relieve the clinical symptoms of the patients with mercury poisoningassociated nephritic syndrome.

ABSTRACT

OBJECTIVE: To explore the effect and significance of tumor necrosis factor-α( TNF-α) in serum and bronchoalveolar lavage fluid( BALF) of coal workers' pneumoconiosis( CWP). METHODS: Twenty-eight cases of CWP were selected by purposive sampling method and divided into 3 groups based on different stages of pneumoconiosis. There were 7 cases in stage Ⅰ,10 cases in stage Ⅱ,and 11 cases in stage Ⅲ. The venous blood and BALF were collected.TNF-α in the serum and BALF were detected by chemiluminescence assay. The lung function was determined. RESULTS: In the serum,the TNF-α level of CWP patients was positively correlated with the CWP stage [Spearman correlation coefficient( rS) = 0. 843,P < 0. 01],and negatively correlated with the forced expiratory volume in one second/forced vital capacity( FEV1/ FVC)( rS=- 0. 503,P < 0. 01). In BALF,the TNF-α level of CWP patients was negatively correlated with the CWP stage( rS=- 0. 654,P < 0. 01),and positively correlated with the FEV1/ FVC( rS= 0. 432,P <0. 05). The TNF-α level in the serum was negatively correlated with the TNF-α level in BALF( rS=- 0. 561,P < 0. 01).CONCLUSION: With the development of pneumoconiosis,the levels of TNF-α in the serum and BALF showed different variation,and were correlated with the changes of FEV1/ FVC. The level of TNF-α could be used as a reference index for evaluating the severity of CWP.

ABSTRACT

To investigate the distribution of mitochondria DNA 4 977 bp deletion, a common deletion (CD), in normal populations of Chinese, human peripheral blood samples from sixty healthy donors were collected, and levels of the CD in genomic DNA from the samples were detected using real-time PCR. The results showed that the CD was found in 27 health donors, with its positive rate being 45% (27/60). The CD ratio was between 0 and 0.00308%, and not affected by age and gender in sixty healthy donors. Our studies indicate that the CD ratio is low, and do not show the age-dependent accumulation and any gender difference in peripheral whole blood from the normal Chinese population.

ABSTRACT

<p><b>OBJECTIVE</b>To measure the peripheral serum levels of CC-chemokine ligand-18 (CCL-18) in patients with pneumoconiosis, and to investigate the feasibility of the index asa potential biomarker for pneumoconiosis.</p><p><b>METHODS</b>Seventy-seven male patients with pneumoconiosis (stage 1:40 cases, stage 2:22 cases, stage 3:15 cases), including 42 cases of silicosis and 35 cases of coal worker pneumoconiosis, were enrolled as subjects, and 162 healthy male physical examinees in our hospital were used as controls. A fasting blood sample (3 ml) was collected from the peripheral venous blood of each patient or control. The CCL-18 concentration in serum was measured by enzyme-linked immunosorbent assay (ELISA).</p><p><b>RESULTS</b>The serum CCL-18 concentrations of the patients with pneumoconiosis were significantly higher than those of the controls [(116.70 ± 82.85) ng/ml vs. (83.34 ± 64.83) ng/ml]; (Z = -2.389, P < 0.05). The serum CCL-18 concentrations of the patients with silicosis were significantly higher than those of the patients with coal worker pneumoconiosis (147.02 ± 93.32 ng/ml vs. 96.43 ± 47.19 ng/ml; Z = -3.030, P < 0.05). There were no significant differences in serum CCL-18 concentration among different stages of pneumoconiosis (P > 0.05). The degree of respiratory impairment was positively correlated with the serum CCL-18 concentration in patients with pneumoconiosis (r = 0.611, P < 0.01).</p><p><b>CONCLUSION</b>Serum CCL-18 level can be used as a potential biomarker for pneumoconiosis.</p>

This paper introduces a display module which can be used on multi-mode medical images. The module has a small size and can be easily used for point-selecting puncture diagnosis and treatment, and registration for image fusion control points.
